Inserting a health information privacy to the form

    I have to build a patient intake form. It must include basic patient information, the Mississippi hipaa health information privacy law or act, and a disclosure form that covers me as the holistic service provider. Is there anyway you can help me with this at all and it must be digital. Its for a class assignment, I have turned whats on file 5 times and each time my professor has kicked it back, so I didn't place the hipaa in the form can someone please help me with this. Thank you

    My understanding is that you want to insert a Terms and Conditions or Privacy field in your form. I checked your form and I can see that you are using the "Text" field to insert a static content.

    I suggest you try using one of these widgets:

    1. Terms and Conditions widget
    2. Short Scrollable Terms widget
