    Only the left half of my form is showing up on my website. The other right half is all white.

    In order for you to show the form completely , you would need to adjust the dimensions of your own form in the generated values width, margin-left, height & margin-top attributes, see this example :

    width: 650px;               /* width of form */

    margin-left:-325px;     /* half width of form */

    height: 450px;              /* height of form */

    margin-top:-225px;     /* half height of form */

    Try re-embedding your form using iframe code:

    <iframe allowtransparency="true" src="//form.jotform.us/form/22420647761149" frameborder="0" style="width: 650px;height:465px; border:none;" scrolling="no"></iframe>

    I did this and it worked.  Now I am just not able to scroll down the whole form! Thanks

    Can you try changing the scrolling attribute value to yes?
