Copied Template - Original name shows in web browser bar

    How do we modify this? 

    The original form name is circled in yellow and the new name of the form has a blue arrow.

    If you want to change the text being displayed in the browser page/window, you can do that by following the steps below:

    Click Settings-> Click Form Settings-> Click the Show More options button

    And than,Scroll down and search for the Page Title->Enter the preferred title and that's it.

    Please follow eezgikrtl's guide to change the page title of your form. That should correct it.
