    Cant log in ...password reset and now I do not have access

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I have checked your account and see that two forms are collecting passwords. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.  

    In order to activate your account, it is required for you to agree with our terms and agree to delete the guilty fields on the forms. Please confirm so that we can delete the guilty fields on the form and activate your account.
