The issue appears to be related to the outdated email templates on your form.
In order to fix this:
1) Delete the current email notifications and autoresponders from your form.
2) Add the new emails.
- Setting up Email Notifications (usually emailed to the form administrators)
- Setting up an Autoresponder Email (usually emailed to the form submitters)

You may also want to delete the conditions with errors.
