Email is not showing the question.

I have a form with rated questions. In the auto response emails the answer is there but the question is not. It is in the attached pdf file.

I have been checking your form and noticed the labels are indeed missing in the email notifications: 

I would recommend you to manually add the labels on your email template. You could also turn on the "Update Email" option on your form so the changes made on the builder. 

The option can be found under the advanced tab:
