my form will not allow people to make multiple entries once they have submitted a formAlso, my 3rd party google doc info is not receiving info from jotform for only one field (location preferred)

The form is currently configured to accept only one submission per unique E-mail address. To fix this, change the 'Unique Question' to the default 'No Check' option.
