Google Spreadsheet is not receiving info from one form field

Also, my 3rd party google doc info is not receiving info from jotform for only one field (location preferred) 

Since you use an Address field type for the Preferred Location field, it is forwarding a sub-label name instead of the field label to the Google Spreadsheet.

It is not safe to change the column name on the spreadsheet.

If you need a single line input to accept the address, you may try a Short Text Entry field type.
