Survey Form Submission Failure

Hello there!

Just created a form and tried to test it on my end. When it got to the last page, instead of displaying the Thank You screen it just shows no forms and a "back" button but that's it.

Can I have some help?

I checked your form and noticed that on the last page, indeed there is a blank area. You can either click the remove page button or remove the next page button to remove that blank canvas

I also would suggest to add a submit button at the bottom of your last page as well.
