Submission files being duplicated every time I edit it

This is to report that the submission files duplicates everytime I make an edit. This is the first time that it happened and it never happened before. I am hoping if you can identify the cause of the duplications.

We had an issue regarding on duplicate submissions when being edited but our developers already fixed the issue. Editing submissions will now update the current submission instead of creating a new one.

Let us know if this is still not the case and we will be happy to check it further.
