My account was suspended.  These are forms for my bouncehouse and trucking company.

Your account has been suspended because of the form whose unique form ID is 71318206086150. It contains some phishing fields. 

"Last 4 of SSN" field is against to our Terms of Use. 

"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

If you agree to our Terms of Use and allow us to delete this field, we are able to activate your account again.

We can take that off. Not needed. But we will need to add
Date of birth and drivers license number. We are a trucking
Company and will need to check driving records.

I have removed the guilty field and re-activated your account. You can now use your forms and manage your submissions properly.
