When I try to convert my form into a pdf. format to be filled out that way when I download it to my computer it is almost completely un-usable. I want to be able to send it to people in pdf format to fill out that way too. Please help!

Do you need the PDF to be filled online, or do you need it to be printed/emailed back to you?
If you do not need an active (fillable) PDF, we can print your web form into a PDF file. It should be easier than the PDF generation.

I need a foldable pdf version of the form so I can send it to people as an attachment. Just looking for two ways to have people fill out the app.

I have just tested with one of your forms and noticed the PDF is generated properly, here's a screenshot of the file I downloaded:
Is this not the same you're seeing on your end?
Please open the form using Adobe Reader, you may also provide us some screenshots about how the form displays on your end, this guide will help you uploading the images: https://www.jotform.com/help/438-How-to-Post-Screenshots-to-Our-Support-Forum

With Adobe reader it works a lot better but the sub-labels are not showing up along with the place holders. Is there anyway to include these?

I believe placeholder text might not be possible to include but the sub-labels should be. I will forward this to our developers to have a look. We will let you know as soon as we have any updates.
