I did check your form but could not locate any form calculation widget added.
You need to edit your "Form Calculation" widget and enable "Add field to equation as text by default" options. Please check the screenshot below:

The field is called Award Text Calculation, and I have already completed the step above.

I checked your form and I am not quite sure what you are trying to insert in the calculation:
It looks like the calculation field is being used in the calculation.
Let us know what text you are trying to combine or display and we will be happy to help get things adjusted.

Hi David,
let me give you some more details.The purpose of this form is to submit award nominations, and pay for the cost of the award + shipping before we print and mail award certificates to the form submitter.
After the form fields are completed, and before the form is submitted, the submitter will visit the "Certificate Proof" page and approve the information included on the design. (See here) Previously when I was using the "Which award will be presented field?" calculation values feature, the Certificate Proof was showing the calculation value, not the title of the award. (See here)
The reason why I have a calculation value for each field is because each award has a different cost. The cost should also be calculated with the shipping cost and cost of any add-ons before the submitter is charged via Stripe.

Thank you for the clarification, I see what you mean now:
When calculation values are present, those values are what will be used in the form calculation, even when set to text.
The quickest work around I can see would be to set up a short text entry field and use conditions to update that field with the appropriate text:
One condition for each option.
