Create PDF not working when using new form layout

I'm trying to create a PDF of the form "Breckenridge Backstage Theatre - Board Application." I've tried several times with both Chrome and Safari, and the little wheel just keeps spinning and spinning.

Unfortunately the new form layout does not support yet the Create PDF form feature. You will have to switch the Form Layout to Legacy layout first the create the PDF for the tool to work.
