If somebody tried to fill the form https://form.jotformpro.com/50434114046948 in a mobile device (we have tried in an Ipad, iPhone 6 and 7, Samsung and Asus smartphones), we cannot fill any field, but go straight to the Youcanbookme widget.
This is a great deal for us, because this is how many clients come to the organization. Its is necesary to fix this AS SOON AS POSSIBLE, PLEASE.

I tested and I was able to replicate this problem.
The issue is related to how you embedded your iframe for this page:
You should remove that field from your page and use iFrame embed widget instead:
In the widget's URL you will copy the src part of your iFrame (the yellow part):
<p><iframe style="height: 1000px; border: 0px; background-color: transparent;" src="https://visittocgb.youcanbook.me/?noframe=true&skipHeaderFooter=true" width="600" height="300" frameborder="0"></iframe></p>
I tested and it worked fine on my iPhone.
