We want to integrate JotForm elements in to an independently developed web application. It's a permission form which we would a name and location field to fill the form (this would be different for each user) and the parents would sign the form electronically. Is this possible with JotForm?

You can embed the form to your webpage via the embed script code, iFrame code, source code, pop-up or lightbox code. Please check these guides:
