    Hi,

    That is correct BUT the box needs to be only a touch bigger than the date etc NOT 100% the width of the form. 

    Also

    I see in your supplied screen shot that the YES / NO is to the left ?

    I added your supplied code to make it center, why does it look like it does ???

    }

    The Yes/No in screenshot was from the builder. Please check if the position of Yes/No is ok or not.
