I would like to put options to register for an event that will take place in two locations, so I would like to give user the option:
1. Louisville Event
2. Lexington Event
3. Both Events
Once they choose 1, 2 or 3 - then I want to give them the option to pay by check or credit card. Option 1 & 2 will have the same prices, but Option 3 will have different prices. Can you tell me the best way to set up this form?

It's possible to achieve that, first you will need to list the options using either a single choice or multiple choice field, this will depend if you want to allow your user to choose more than one option or not.
