Hi,
I am creating a form for a client and they want their clients to initial each point so they know if it understood and agreed upon.
Can this be done in Jotform and if so what widget etc would you recommend.

To my understanding, you trying to create a form where the respondents would need to put their initial on each question/field to indicate that they understand and agree to it, is that correct?
I'm sorry but I'm not sure I completely understand what you are trying to achieve, but you can add any of our signature widgets in the form and set it to required to make sure that it will not be left unfilled.

Yes, I need to create a form where the respondent puts their initials into each question or field.

I did check your last edited form and found that you have correctly added textbox question for user's initial. Please check the screenshot below:
If you want, you can add any of the following signature widgets in your form instead textbox so that users can actually sign in form:
