I have been testing your form and can see what you're reporting. It seems like the issue is related to the connection the widget has with your API.
May you please try deleting an adding the widget to your form?

You mean deleting the current checkbox and adding another one?? If not, could you walk me through it? like step by step for dummies?

Yes, deleting the current widget should resolve any issue with the current one. To delete a widget you need to edit your form in the form builder and select the widget, once selected click on the delete button:
Add it once again to get a fresh version of the widget including the API key:
