i cant open my form

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I have checked your account and see that there is a form which is collecting Wells Fargo username and password which is considered as a phishing form. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.
It is required to delete the form in your account in order to activate it. Please confirm so that we can delete the form and activate the account.
