How can i create a conditional form in JotForm
Eg: Radio button - choice is DD or NEFT
if they choose they should just get the fields for only DD.

First of all, please see this guide which will help you on how to create smart forms using conditional logic: Smart Forms Using Conditional Logic.
You can set up your form to show multiple fields base on the user's answer.
Here's the guide on Hide or Show Multiple Fields at Once.
I have created a demo form for you based on your requirements. You can clone this form: https://form.jotformpro.com/72702422450951 to see how it was set up. Here's the guide on How to Clone an Existing Form from a URL.
