I have some questions about JotForm. How can I get assistance?

Hello, 

I am looking for an application to build a form for submission of continuing education course materials that also includes completion of a W9 and an electronic signature. The product must also include API integration to funnel the data into our learning management system. Please contact me via email to setup a demo and answer my questions.
