Hi, we'd like to see if we can create a fully customized URL for a published form? Not opposed to upgrading to a paid service if that means it's possible. Basically, we own the openapproach.com domain, and we'd like to publish a form to one of our web pages (not yet created) that is maybe openapproach.com/forms/new_employee so that it's straightforward for people to get to. Is that possible, or do you only offer the ability to use the jotform.com/%username%/custom_form_name customization?

You can customize the end of the URL as you have already seen quite easily:
