My account is suspended, please re-open my account

    Hello,

    i am Dr ,Mohamed abdelrazik  assistant professor of surgery i am using your great jotforms since long time with differnt ways paid and free with my other accounts

    My problem : 

    I made a form two  days ago to help in approving  the research committee in my university and i embed it in our official website as a trail till we pay for the jotform subscription  but today unfortunately it was disappeared and the account (mahraga7@hotmail.com) was suspended 

    please re open my account

    I checked your account and found that one of your forms is asking user's password and since this is not comply with our terms of use, you account has been suspended.

    If you want your account to be active again, you need to allow us to remove that sensitive form field from your form.

    As per your instruction, I have removed the password field from your form and reactivated your account.
