The jotform that should work is this link:
and we've printed 10,000 brochures with that link on it, and now it's not working....HELP! We can't reprint, and we have gotten no submissions in the last 5 days. The form is called "Saturday Literacy Academy" and is in my account: Mary Ann Cahill.
Please advise as soon as possible.

Hello Mary Ann. I have checked the form and it seems to show the form on the said URL. Are you still experiencing problems when viewing the form on that link?

People would need to enter your form's URL in the browser's address bar, NOT the search bar.
In any case, I perfectly understand where you're coming from. Although I don't think we are in the position to order Google's search engine bots to crawl your form, just so you know, I have used Ping-o-matic to alert web directory services to index your form.
It may or may not cause your form to get indexed, but that's the best I can do to help you with your issue. It might take a couple of days to see the result, though.

Hi macahill, In addition to my colleague's response, rest assured your page opens fine when the URL is pasted onto the address bar.
I tried to open the form with a number of browsers (Chrome, Explorer, Firefox) and it has worked well for me.
Here is a screenshot with the URL you have given using the three browsers.
