How can I create a form that the buyer can purchase more than one product?

    How can I create a form that the buyer can purchase more than one product? Every time I click on a second product, it unchecks the first. Am I using the wrong template?

    I checked your form and it is allowing for the selection of more than one product:

    It also looks like you are able to set up quantities and other options.
