I am attempting to convert my jotform into a fillable PDF file and it has strange markings all over it and it doesn't look very professional at all. It also isn't bringing in our headers. Please advise.

Note if using the legacy layout:
1. Remove all CSS code (if there is any)
2. Make sure all fields have not enabled "shrink field"
3. Make sure all fields have not enabled "move to a new line"
4. Remove widgetsThis will create a vertical form that shouldn't overlap

Thank you so much but I am not understanding what CSS code is and it is a must that we have "shrink fields" in order for the form to flow properly with our questions. I am not familiar with "move to a new line" either. Or widgets. Please advise on how to create a readable, understandable PDF from our current form that won't have strange color boxes and symbols throughout. It may be helpful for you to try to recreate what I shared in my above question so you can better assist me in getting this done. Now, in order to get the fillable PDF to work we need to do all of the following:
1. Remove all CSS code (if there is any)
2. Make sure all fields have not enabled "shrink field"
3. Make sure all fields have not enabled "move to a new line"
4. Remove widgets
5. Remove theme (if enabled)What I suggest is that you first clone the form you want to convert to a fillable PDF. And with this copy you can apply the modifications. That way you keep your formatted form to distribute online. And use the cloned form to distribute offline.
How to Clone an Existing Form from Your Account
1. To remove all CSS Code:
Click on the form designer ( blue paint brush icon ) and on the CSS tab make sure the box is empty.
2. Make sure all fields have not enabled "shrink field"
Open the properties of the field ( blue gear icon ) and on the advanced tab disable shrink.
3. Make sure all fields have not enabled "move to a new line"
Move to a new line only appears if shrink is enabled. So we don't need to do anything for this step because all our fields should have shrink disabled.
4. Remove widgets
To delete widgets please read this guide:
How to Add a Widget to your Form?
5. Remove theme (if enabled)
To reset the theme you need to open the form designer ( blue paint brush icon ). And on the themes tab select the default theme.

I spoke with you on chat about this and even when making the form format what I believe should translate properly to fillable PDF format, it is still not properly generating the PDF:

Hey David, what about a hard copy NON-fillable form that we can use to mail to someone? Is that one possible?

That is actually quite easy to produce. Add a print button to the form:

Then use the "Print to PDF" option after clicking print.

yes, but you have to fill out the form in order to hit submit, so how can i print out a blank form to mail to clients who don't have computer access?

No you don't have to submit it. You can simply print the form empty.

But it won't let me move forward through the form to get to the print button unless I fill out the required buttons on each page first. Maybe I am confused??

Ok, so if you just want to print an empty form, in the builder click on the last page then hit preview. It will take you to the last page where you can print your form.

No, that doesn't work either. I go to the last page on builder...I click "Preview Form" then it takes me to the form and I have to fill out our required info in order to get to the next page....it takes me back to the beginning again. I'm not understanding why I am unable to print a blank form....
