Account suspended

    Hi one of my account has been suspended. I would like to know the cause and what changes can I make to lift the suspension.

    That account was suspended due to requesting sensitive information like social security numbers.  If you can provide proof that you are an authorized mortgage/load provider, we can unlock the account. 

    Please contact our support staff at support@jotform.com from an email address under the authorized business' domain.  That was we can verify not only the business but your affiliation with that business. 

    If that cannot be done, then the account will remain closed.
