How to reactivate suspended account

Hey my account is suspended as well. Can't login to submit ticket. Can you help with this please?

We would like to apologize for any inconvenience. I have checked your account to see what triggered the suspension and found these forms — http://www.jotform.us/form/72604531489157 and http://www.jotform.us/form/72714379284162

Both forms are identical since based on the title of the second form, it is a clone version of the first one.

I have checked both forms and found the following fields:

Please be informed that those are sensitive information and we do not encourage our users to collect such information in our forms. Collecting sensitive information violates our terms.

Here's an excerpt from our Terms:

We will be able to reactivate your account provided you agree that we delete the form fields in your forms.

We will wait for your response.

You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

The site the form is embedded on contains an SSL Certificate. It was using the secured form feature and I would be happy to use the encrypted form feature as well once the account is reactivated. 

I researched the forums and terms and read that collecting this information was allowed and that is why i chose jotform and It doesn't appear that I am in violation of the terms and policies.

I checked your account and see it is currently active. I also see that you are using an encrypted form as specified in our terms of use
