I did check your account "RLI" and did not find any issue with that. Your account is already active and you should not have any trouble.
Are you unable to login to your JotForm account? Do you see any error message when you try to sign in? Please share a screenshot of the problem and we will take a look. My name is Ian Reeves I own a Registered Training Organisation (RTO). I do
ask on the enrolment form for the drivers licence number to confirm my
students identification. A licensing requirement.
I train and qualify students in a number of heavy vehicle driver licences.
Our Motor Vehicle Registry (NT Government Office requires some licences to
be verified).
I am not aware that I requested a SSN on the enrolment form it is not an RTO
or regulator requirement. If it is on the form I will remove it.
At the moment I still can not access my account nor can future students
enrol into any courses from my website www.logictransporttraining.com.au
It was never my intention not to follow you policies .
Please let me know what I am required to do to have the account enabled to
allow students to enrol into courses.

Based on your previous reply, you were able to login to your account RLI. The status of the account RLI is Active and you should be able to log in and access your form properly.
Are you referring to a different account? I checked the site you provided but I was not able to find any JotForm embedded to the site. If you can provide us the direct link to the form or exact URL where you have embedded the form that you mention you cannot access so we can further check.

I am over all these emails from different members of your team say that my
account RLI is active Because it is notorious you havent whitelisted my IP.
I can not get to any part of your web site. As soon as I enter jotform in
the url I receive the following
I am now loosing customers because because enrolment form is not available.
This afternoon a company rang my mobile to enrol 4 of their staff into a
truck licence course @ $990.00 per person and there is NO enrolment for.
I have been using this jot form enrolment form for about 4 years without any
problems. Now you have banned me for POSSIBLE phishing.
You have my contact detail on your system why not contact me first to
discuss the issue if there is one.
What happened to customer service?
How many more emails do we need to fix the issue.
I have been asked to provide a screen shot. Well I am confused what screen
shot do you want. I can not access jot form in any way what so ever. I can
not provide a screen shot of something that is not there.
I appeal to your support team to undo what has been done. Whitelist my IP,
unbanned me what ever it takes but Please give me access to my account. I
can not edit the enrolment form to add or delete anything until your team
click the undo button.

When I checked your account yesterday, it was active. That's the reason in my first response, I requested you to share the screenshot of the issue.
Anyways, I have gone ahead and whitelisted your IP address "10.240.0.12" from where you last logged in successfully to your JotForm account. Have your IP address changed? Please let us know your current IP address and we will whitelist it too. Please click on the following URL to know your IP address: https://whatismyipaddress.com/

Please note that we have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities. Since the anti-phishing system is automated, your account was suspended automatically. One way to solve this problem is to upgrade your account to paid plan. Please note that paid accounts are not suspended automatically without manual verification of forms.

Thank you for responding . As I could not get access to my account my
original IP I had try from an external IP.
If you would be so kind to check and whitelist my original IP 203.214.22.47
it would be greatly appreciated.
At this point in time I am only using the form from Jotform and that is my
enrolment form. As things settle down at this end I will consider the
upgrade to a paid account.
