How can I setup a field with 1 question and 4 answers?

Hi,

I need to request test results from the client but I don't want them to be in separate questions.

For example, the question will be "Test Results" and just like the full name element, I need 4 sub-labels which will be writing, Reading, Speaking and listening.

None of the regular form fields would achieve this so using a widget might help. The widgets I'm listing below have different options for multiple entries. This would mean that for a single question, you can set it up to have 4 fields that your users can fill out.
