I have just checked your form and found that you are hiding the page with two different conditions, please note that this may be generating a conflict since if a condition is not met then the page will be automatically shown.
I would recommend you disable any of the conditions or fix the conflict.

Please apply the following steps:
1. Disable the second condition that hides the page 5 of your form:
2. Add a short text entry field that will serve as flag so both conditions can work at the same time:
3. Now add a condition to your form like the following:
This is basically the same condition you had before, but instead of hide the page it's inserting a value on in the flag field.
4. The last step is to include such field in the condition you noticed that does not work:
This way both conditions should hide the page 5 of your form. Here is also a cloned form of yours where I have applied this and it worked: https://form.jotform.com/72768359312969
