My login has been suspended.

My login has been suspended and i am also not being able to access my form. Can you please help me with this as soon as possible.

Your account rahulalapati43 is suspended, because of the Police Incident Report Form. Inside of this form, you are asking for the user's login credentials (username and password) and that is in violation of JotForm Terms of Use.

Phishing - You agree that Company may terminate your Jotform Service immediately If a form is created or Jotform is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately. 

You also have a field that is used to collect Social Security Number. 

In order to activate your account, it is required to agree with our terms. We will also have to delete the username and password fields. 

Regarding the SSN - If you want to collect SSN you need to add a signature and terms & condition fields to your form, so your customers will agree to provide their SSN along with personal info.
