We are trying to send email confirmations for orders and they are not including all the dates.
There seems to be no way we can put form fields into the emails but guessing what they are.

It is possible that one of these field names are incorrect. This is the reason why fields do not appear on email.
The easiest way to fix this is to recreate the autoresponder notification. Delete the existing autoresponder and then create a new one so it would generate all the correct field names.
