Hidden fields is showing in my form

When I hide the fields, as desired, the calculation for Individual ID does not work.  What should I do about this?  Does it make a difference if I hide the field using CSS rather than directly hiding it?

This is the case when the form is accessed directly, not through the prepopulated URL.

The URL is the same as the first entry in this thread.

the fields presently hidden in that form are the fields that should be hidden.

the fields should be hidden anytime the form is in use, except as indicated by the conditions.

At this time my in office testing indicates that I have the form working correctly.  I need to have it tested by others in my team before I am certain.  Thanks for your help.
