Is it possible to create a PDF form? Church Nursery Registration Form

Hi. We are possibly interested in putting our nursery school with church nursery registration form into jot form but want to understand how the downloadable PDF will populate and if it is customizable? 

Also - can we build question logic into the application? (for example, if yes, then next question (#2) shows, if no, skip next question and go to question 3)
