One of my forms is providing duplicated responses

    I've made a form based on a clone of a previous from we've used as a booking system for an event we're hosting. The form more often than not receives responses as two or even three entries. The person entering the form will only have submitted once but multiple responses will come through, all with the same answers and some even being submitted in the same second as the original.

    At the moment I am going through the submissions box daily and deleting the duplicates.

    The form uses the "appointment slots" widget.

    Any ideas as to what could be causing this problem?

    I did a test submission in your form but I wasn't able to replicate this issue, only one submission is showing:

    Is your form embedded in a website by any chance?

    Hi Nik

    The form isn't embedded anywhere and has been distributed by email. The submission posted just after yours has repeated the issue, one booking was logged as a single booking as intended, but the second one has again doubled itself. It's odd as this issue seems to be hit and miss, sometimes it's one entry, sometimes two and occasionally three.

    I've cloned your form named "Learning Conference Bookings 2017" on my side in order to test it. I am not able to replicate the issue you have mentioned on my side. 

    Besides, I have fulfilled and submitted the form on your side twice and my test submissions did not duplicate as seen below:

    I didn't encounter any issue with the form both on your side and my side. 

    Besides, please be informed about that, if the Thank You page is refreshed, the following alert will appear, if your users click on RESEND or CONTINUE, they will send the whole submission again:

    I think some of your users might have been refreshed the Thank You page and resent the submission again. This could be the only reason of duplicated submissions. 

    My recommendation to avoid this error could be setting unique submissions to the form as you can see in the following guide:
