Why is my form cutting off on some screen sizes?

    This form look great on some screen sizes, but in others- part of the form field gets cut off- including the submit button.

    How can i fix this issue?

    I have been testing your form and it seems to be displaying properly even on mobile devices:

    I can see you have added the mobile responsive widget. Are you still experiencing the issue on your end?

    can you check tablet versions?

    I checked on Galaxy Tab simulator, but I wasn't able to replicate this issue, the form is showing fine:
