My account was Auto Suspended. It looks like it is due to using the word "Password" in my form.
I can get rid of that field and was considering using it so that I could make sure the right submitter was entering and no one picked the wrong name.

I checked your account and your account is currently active. You should be able to access your forms and account properly.
