Account suspended

    Hello, 
    My account has been auto suspended. From looking at your support section it looks like this can occur if some part of your terms and conditions has been violated, such as requesting certain personal information. I am not sure what may have prompted this in the forms that I created so can you please check to see what the issue may be? Thanks so much for your assistance.

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I see that your JotForm http://www.jotform.us/form/72526709284160 is collecting Passport information which is why the account is disabled. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

    In order to activate your account, it is required for you to agree with our terms of usage and agree to delete the guilty fields on the form. Please confirm so that we can delete the guilty fields on the form and activate your account.

    Thanks for the response. I agree to the terms and conditions and please go ahead and delete the guilty fields and reactivate my account. Thank you!

    I have deleted the guilty fields and re-activated your account. You can now use your forms and account properly.
