    I was unable to find any JotForm account related to any of the email addresses you have provided. I have searched using the domain and there are a lot of accounts, may you please provide us with either the Jotform username, email address you used on your account or a link to a form in your account? 

    We will be glad to help you finding the account and resetting the password.
