How to create custom smart forms for healthcare?

    I would like to know if you are able to help me form design for use in my dental office. We need a template to be able to complete a series of If/then type entries for each patient we see, and also be able to use multiple choice or drop down options for each section of our form. I also want to be able to view it in the format of an online app that progresses after each question is answered. Can someone contact me to go over more specific details?

    I have seen that you have a guest account. Firstly, I suggest you open a Free JotForm account at the following URL in order to experience JotForm much better: www.jotform.com/signup

    You are able to create any form as you wish by dragging and dropping form elements, you are able to design them by using themes or injecting additional CSS codes.
