The picture was sent to me by one of our customers as to what she sees when she clicks on the link. I see the full survey as can my colleagues on our work PCs but not on our own devices or laptops from home.
Please advise resolution asap.

I was able to replicate mentioned issue when I used the link you provided.
I cleared your form cache and the issue should be resolved now.
Please tell your users to try again.

is saying on the final page in bright red
There are errors on the form. Please fix them before continuing.
Please advise asap - customers are still trying to use the form and this is
creating us some severe embarrassment.

Is the issue with a blank page resolved? Considering that your other issue is not related to the subject of this thread I move your other issue to a separate thread and we will provide you an answer there shortly.

How can I be sure that this error won't happen again when I reissue the link to our customers?

It looks like it was a temporary issue with the form caches. You may also clear the form caches by following the steps provided in the guide below:
