Creating order form for Hospital

    Hi there, I came across your site and love how easy and integrated it can be.

    I am trying to use if for a client that I have (i own a small business) to put on their intranet site or have a portal that they can fill this form out to order a piece of apparel for their employees, actually each employee is responsible for order their own so this seems like it could work well.  I have already gone through a round of orders and I am about to open another ordering window.  

    I am concerned with hidden pricing - I have evaluated the pricing structure but want to make sure I understand.  

    I think the 2nd package would work for me, I dont think I would need more than 100 submissions monthly but would want to maybe store original copies (for some period of time) then would want to create a workflow (thinking of using Zapier) to automate once submitted to send email to me as well as upload to google docs and open Trello ticket.

    So does this sound like I have it correctly -- send Dir. of IT at client link for intranet site, push live, let employees submit own forms (no payment through jotform), once submitted info would be emailed to me or I would be notified that there was a submission.  Besides, submissions, storage of submissions, and views, am I missing any potential monthly payments?

    We do not have any hidden fees. We also offer monthly, yearly and bi-yearly billing cycles.

    The Email, Google Drive and Google Spreadsheet integrations are available at JotForm. Zapier is supported too.

    Are the integrations easier going through JotForm than pulling in zapier?
    Trying to keep it as simple as possible — and sorry by “hidden”
    I meant was there something I was missing ?

    Yes, the native integrations are usually easier to setup and use. They are also being triggered immediately upon the form submission.

    I do not think that you are missing something, monthly form views and monthly submissions are pretty much the main limits that you should usually consider. We reset the limits on the first of each month.
