PDF attachment not sent with autoresponder email

I have created an autoresponder email for my jotform form.  I used the advanced tab to upload a pdf page to attach to the email. When I send a test email to myself the attachment is not present with the email. How do I make sure the pdf is sent as an attachment with the autoresponder email?

You have to preview the form and submit an actual test instead of using the Test Email button. The test email button is designed to only send the defaults in the email body.
