This has historically been a problem and has once again resurfaced. Large file uploads are failing from multiple browsers, on multiple speed connections. My form has not changed and I have received uploads of 700+mb for months without incident. Please look at my history before telling me something is setup wrong.
The form gets to 5-10% and then errors out with a timeout message.Page URL:

I will try to test the upload form on my end. This may take a while though due to upload process. I will get back to you of the result.
I understand that your concern have historical basis on it. So, I can only try on my end if I can reproduce that the problem have resurfaced, after it had been previously reported to have been 'fixed' already.

My test upload was successful using a clone of your upload form. Please refer to series of screens below when I monitored its progress...
I then tried saving the attachement back to my computer using the form's submission page. Just to verify if complete file was uploaded.
I can conclude at this time, that there is nothing wrong with your form and the upload field process do work even at files greater than 600MB.
Perhaps the upload failures that you can confirm that had happened, was probably due to other reasons.
Maybe you can also add some information on your forms, informing the would be user that in case the submission was not successfully process, they should fill-up another form (a simpler form) and make them state time/date and error they encountered during the upload/submission. This way you will be able to capture important informations from your form users, when this happen.

The problem with larger uploads is now resolved.
