I was able to replicate mentioned issue. I made some test and I found that the Adobe Sign widget is causing this issue. I will report this issue to our developers and we will inform you via this thread once this issue is resolved.
For now, I can only suggest you to remove Adobe Sign widget from your form. We have other signature widgets that you can use.
