How to show an image in the form?

    Hey Hey Hey ! 

    Trying to find a way to make a "proofing/approval" form for creative assets.

    Would like to integrate that asset (usually an image) onto the form, and have a Approve / Refuse button & a E-Signature field underneath it

    It also would be cool that in the submission pdf we do see that image and the signature.

    The image would come from another form that i will make to send for approval - idk

    Would you be able to help me out ?

    You can integrate an image in your form by using the image field:

    Then you can use radio button field to approve / refuse:

    And you can add signature widget from the panel:
