How can I add an image to a question?

    How can I include an image related to a question? I tried inserting an image, but it becomes a separate "question." I'm including a screen shot that hopefully explains what I mean.

    Your form is created using the New Layout and with the New Layout, it is not possible to display multiple fields on a single page. 

    Maybe you can use Draw on Image widget or Input Table field.

    Another solution is to change the layout of your form to the 'Legacy Layout'. this can be done on the Form Settings page.
